The following requirements must be met before research samples will be accepted by Specimen Control:
All requisitions should be stamped with the patient’s addressograph card.
No other information is needed in the upper right hand corner of the requisition.
Use of a University of Iowa Hospitals and Clinics number on the requisition will send all results to the patient’s medical record. All charges will appear on the patient’s bill and Special Programs will extract the research charges to be billed to your research account.

No other information is needed in the upper right hand corner of the requisition. However, it must be stated on the requisition in the Miscellaneous Testing section that Patient Name must be entered in the computer for each test ordered.
Use of a Research 10 account will send all research results to your 10 account. All charges will be billed by Special Programs to your 10 account.
All requisitions should be stamped with the enclosed addressograph card which has been issued for your research project. The following information is required on all requisitions.
Use of a University of Iowa Hospitals and Clinics number on the requisition may result in patient billing unless the appropriate research project green card has been used to stamp the submitted requisition. If a patient is inadvertently charged instead of the research account, a Request for Research Crediting form must be completed by the Principal Investigator and hand delivered to the supervisor of Specimen Control to remove the charge from the patient’s account. Although a medical record number is an acceptable patient identification when used in conjunction with the research project number, a subject’s specimen can alternatively be identified by subject’s name, social security number, or other unique identification assigned by the researcher. Non-human specimens should be identified as such on the requisition.
