* GCRC applications for industry-initiated protocols should include a copy of the Informed Consent Document
sent to the Western IRB and a copy of the Sponsor Protocol.
GCRC Application Directions:
1. You must have MS Word 6.0 or higher or a program to read these
files.
2. Use your mouse to select "GCRC Application", then save
it in the appropriate folder for later use.
3. When you are ready to complete the GCRC application, open the file with Word and use
your arrow key or tab key to move from line to line or box to box to complete the required
information. When you "click" on some boxes or press the space bar to
"choose" them, an "X" will appear when they are chosen;
choosing them
again removes the "X".
4. We recommend you do not unprotect these forms as the results are unpredictable.
The GCRC Application form contains routing information for your protocol.
