  • Family Medicine Faculty Development Fellowship: Instructions to Applicants

    • Completed UIHC GME Application Form (forms available from Linda Hoover)
    • 3 letters of reference (current UI FM residents may use letters on file from residency application)
    • Acceptance in U of I Carver College of Medicine Masters in Medical Education program
    • Written personal statement including background and interest in medical education, educational goals and career goals
    • Interview with Fellowship Director and selected faculty members
    • Applicants should submit all materials by January 1 of the year prior to desired enrollment.
    • Acceptance decisions will be made by the Program Director, who may call upon the Advisory Committee for consultation.
