Application Process Checklist

All of the following applications materials must be submitted BY THE APPLICANT as a single packet. Documentation packets missing any of the following will not be considered. The deadline for submission of this packet is February 1st of each year for the classes that begin the following September and March.

  1. A completed application form.
  2. Cardiovascular Interventional Program's Technical Standards Form (read and sign).
  3. Cardiovascular Interventional Program's Self Disclosure Form (read, sign, and reply if applicable).
  4. Completed Essay based on required questions.
  5. OFFICIAL transcripts from:
    • All colleges and Jr. colleges attended ((UI students must also have theirs sent or contact Ann Kunkel (ann-kunkel@uiowa.edu; 319-335-3838) in UI Admissions to have copies of those at UI sent to us.)
    • All technical and professional schools attended (Coursework taken at foreign institutions must be evaluated for United States equivalency.)
  6. Copy of ARRT examination scores. If you have not received them please indicate date they were taken on application. These must be submitted as soon as score is received and is required before start of program.
  7. Three personal reference forms completed by individuals who are familiar with your personal, academic, and/or employment background. To ensure confidentiality, each recommendation must be individually sealed in an envelope which bears the signature of the person writing the reference across the seal.
  8. TOEFL scores (if applicable). A minimum TOEFL score of 600 on the paper-based test (250 on the computer-based test) is required to be considered for admission. (See Program Information, Admission Requirements.)

The documentation packet MUST arrive at the program director's office by the February 1st deadline. Out of fairness to those applicants who have met the deadline, applications arriving on or after February 1st will not be considered for the upcoming CVI Program unless spots in the program remain available.
